CASS’ clients are assigned a case manager to help develop goals and personal expectations through discussion and assessment of their unique needs. A case plan is then developed to assist clients with obtaining the necessary resources and services to empower them to end their homelessness. Assisting clients in locating stable housing that is appropriate and affordable is an essential aspect of the case plan.
CASS Case Managers are guides to education options, substance abuse support programs, mental and behavioral health services, veteran services, income benefits, employment services and other vital resources necessary to empower clients to end their homelessness.
CASS partners with a wide range of supportive service providers who bring services on-site to CASS facilities or have satellite offices on the Human Services Campus. Some clients are referred to off-site providers within the community (such as specialty doctors) depending upon the individual needs as identified in their case plan.
